The physicians at this practice require a referral from your Primary Care Physician or a physician that has treated or diagnosed you with a Pulmonary problem. Your referring physician will need to fax your Referral Order and pulmonary work-up, which must include up to date office notes, recent Chest x-ray, Chest CT, PET/CT (if recommended), and PFT testing results. Along with copies of ALL Insurance cards, as we are not providers for all Insurance companies or plans.
